A little prep keeps the visit short. If you're not sure what your form requires, call us and we'll walk you through it.
- Photo ID (and a driver's license for DOT exams)
- Your insurance card, or another way to pay
- The form from your school, team, employer or agency
- A list of current medications and allergies
- Glasses or contacts, for the vision screening
- Vaccination records, if your form asks for them
Questions, answered
Do I need an appointment for a physical?
No. Walk-ins are welcome Monday to Saturday. Saving your spot online or by phone holds your place in line so you spend less time in the waiting room.
Will you fill out my school, team or employer form?
Yes — bring the form with you and your provider will complete it during the visit.
Does insurance cover physicals?
It depends on your plan and the type of physical. We accept Medicare, Medicaid, Oscar and Curative, plus cash, credit and debit cards, Apple Pay, Google Pay and HSA/FSA. Call us and we'll help you check before you come in.
Can my company send employees to you?
